Dr. Sawlani received a Certificate of Recognition from the NCQA for providing quality cardiovascular and stroke care to his patients. Dr. Sawlani founded the practice in 2002, finished his Internal Medicine training at Rush/Westlake hospital, Chicago, IL, and was Board Certified in Internal Medicine in 1996.

Dr. Sawlani is a USCIS licensed Civil Surgeon, certified to do immigration medical exams.
Dr. Sawlani has been in Private Practice since 1996. Dr Sawlani is currently on staff at Gottlieb Memorial Hospital, Community First Medical Center, Presence Resurrection Medical Center and also at various nursing homes. He is the Medical Director of Elmwood Care Nursing Home in Elmwood Park, IL as well as Regency Nursing Home in Niles, IL. Dr. Sawlani is also the medical director of the Illinois Physicians Alliance.

Dr. Jablonska is a family medicine doctor at Central Primary Care. She received her medical degree from Medical University of Warsaw and has been in practice for more than 20 years.
